LEA Membership Tracker Training Guides


The LEA Membership Tracker provides real-time notification to local education agencies (LEAs) regarding a student’s demographic and enrollment records. The Demographic Conflict module identifies the discrepancies within a student’s demographic record between the LEA’s Student Information System (SIS) and the Authoritative Database maintained by OSSE. The Exit Management module facilitates the LEA verification of incomplete exits for a student’s enrollment record.

This will reduce the administrative burden on LEAs to conduct multiple data verifications throughout the school year. The LEA Membership Tracker provides accurate and reliable data and will support a more equitable Uniform Per Student Funding Formula (UPSFF) payment approach.

Master Student Demographic Change Affirmation Form

The Master Student Demographic Data Change Affirmation Form should be used by LEA to assist with resolving changes to student demographic indicators within the Authoritative Database maintained by OSSE. This form can accompany or be used in place of supporting documentation listed within the LEA Membership Tracker Guidance Document.

OSSE Enrollment Verification Form

The OSSE Enrollment Verification Form should be used by LEA to assist with verifying a student’s Stage 5 enrollment at an institution. This form can be used for supporting documentation for specific exit codes listed within the LEA Membership Tracker – Exit Management Guidance Document.

OSSE Death Verification Form

The OSSE Death Verification Form should be used by LEA to assist with verifying a student’s death. This form can be used for supporting documentation for a specific exit code listed within the LEA Membership Tracker – Exit Management Guidance Document.

Notice of Intent to Home School Form

The Notice of Intent to Home School Form should be issued to the student’s parent/guardian when it has been indicated that the student will be homeschooled in the District of Columbia.
